Job summary

The Monarch at Brooklyn Rehabilitation and Nursing Center is seeking a reliable Home Health Aide (HHA) to safely transport residents within and outside the facility, while providing friendly, compassionate care during appointments, activities, and therapy sessions. The role requires an HHA certification and a valid driver's license if offsite transport is needed.

Responsibilities include assisting with transfers, monitoring safety, and communicating resident needs to the care team.

This is a vital role that ensures residents arrive safely to appointments, activities, and therapy sessions while providing friendly and supportive care.

Home Health Aide (HHA) Responsibilities

• Safely transport residents to and from medical appointments, therapy sessions, and facility activities
• Provide assistance and support to residents during transfers and mobility challenges
• Monitor and ensure resident safety and comfort during transport
• Communicate effectively with the care team regarding resident needs and observations
• Assist with light resident care and companionship as needed
• Follow all facility policies, procedures, and safety guidelines

Home Health Aide (HHA) Qualifications

• HHA certification in good standing
• Valid driver's license and clean driving record (if transporting offsite)
• Ability to lift and assist residents safely
• Strong communication and interpersonal skills

Pay Rate: $17.50 per hour
