Parkview Care and Rehabilitation Center is seeking a Home Health Aide to assist residents with daily activities and mobility in a respectful and compassionate manner. The role involves escorting residents to appointments, monitoring safety, and providing snacks during passes.

The position requires a current New York State HHA certification and a commitment to resident well-being within a supportive care team.

Qualifications

• Current New York State Home Health Aide (HHA) certification.

Responsibilities

• Escort residents to and from clinic appointments and dialysis sessions.
• Monitor and prevent falls and altercations, intervening immediately.
• Assist residents in preparing for appointments and ensure they receive snacks during out-on-pass periods.
• Report incidents and accidents promptly to the clinical coordinator or ADNS.
• Act as a representative of the facility, adhering to all policies and procedures on and off the property.
