General Purpose:
Members of the interdisciplinary team who provide hands-on personal care, report changes in the patient’s/client’s condition, perform simple procedures as an extension of therapy or nursing services, and provide incidental/instrumental activities of daily living as assigned to patients/clients in the home setting, following policies, procedures, and regulations.
Essential Functions:
- Read and record temperature, pulse, and respirations.
- Assist, perform, and train patients and families in activities of daily living such as bathing, grooming, skin care, oral hygiene, toileting, and nutrition.
- Promote a safe environment and maintain unobstructed pathways during care.
- Assist with ambulation, transfers, and range of motion exercises.
- Assist with incidental activities like shopping, meal prep, socialization, homemaking, and maintaining a clean environment.
- Assist in medication reminding and administration of self-administered medications.
- Follow infection prevention measures, including universal precautions and PPE.
- Perform only tasks assigned per the service plan.
- Recognize, report, and document changes in patient condition and safety.
- Accurately document observations and activities.
- Organize self to complete visits and tasks efficiently.
- Complete mandatory in-services and provide required documentation.
- Participate in QAPI and Emergency Preparedness programs.
- Complete other assignments as requested.
- Respect patient rights and confidentiality.
Home Health Aide MAY NOT:
- Perform skilled nursing procedures prohibited by law or regulation.
Minimum Education & Experience Requirements:
- Must be 18+ years old.
- Six months healthcare experience or completion of the Office's preceptor program.
- Be a nursing student in last year or have documentation of skills, or be a graduate RN or LPN/LVN.
- Complete approved aide training and competency evaluation.
- Or meet state licensure and registry requirements.
- No work lapse of 24 months as a home health or nurse aide; if lapse occurs, re-completion of training is required.
Knowledge, Skills & Abilities:
- Two positive references.
- Pass home health aide knowledge exam.
- Pass criminal and abuse background checks.
- Effective communication skills in English.
- Ability to read, write, and verbally report clinical info.
- Prioritize multiple tasks efficiently.
- Proof of valid driver’s license and auto insurance if applicable.
Working Conditions & Physical Effort:
- Travel within service area.
- Work in patients’ homes.
- Potential exposure to infectious materials, requiring safety precautions.
- Constant standing, bending, reaching, and lifting up to 50 lbs.
