Job summary

Qualifications

• Meets state and federal requirements for Home Health Aide certification or training.
• At least 18 years of age.
• Ability to read, follow care plans, and document services provided.
• Current CPR certification and valid driver’s license with reliable transportation.

Responsibilities

• Assist patients with daily living activities such as bathing, dressing, grooming and toileting.
• Support mobility, transfers, and basic exercises per care plan.
• Maintain a clean, safe home environment and help with meals as needed.
• Take and record vital signs when trained to do so and report changes to the clinical team.
• Document care provided accurately in the EMR system and participate in team meetings.

What You’ll Do

Provide compassionate, hands‑on care that supports patient comfort, safety, and independence:

• Assist patients with daily living activities, including bathing, dressing, grooming, and toileting
• Support mobility, transfers, and basic exercises in alignment with the care plan
• Help maintain a clean, safe, and comfortable home environment
• Prepare meals and assist with feeding as needed
• Take and record vital signs when required and trained to do so
• Assist with medication self‑administration as directed
• Observe and report changes in patient condition to the clinical team
• Document care provided accurately and timely within the EMR system
• Participate in team meetings, trainings, and care coordination activities
• Provide compassionate support to patients and families during each visit

What We’re Looking For

We’re seeking individuals who are dependable, compassionate, and comfortable working independently in a home‑based setting:

• Meets state and federal requirements for Home Health Aide certification or training
• At least 18 years of age
• Ability to read, follow care plans, and document services provided
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
• Ability to work independently with strong time management skills
• Demonstrates empathy, patience, and professionalism in all interactions
