VitalCaring is seeking a dedicated Home Health Aide to provide compassionate in-home care, helping patients with daily activities, mobility, and safety. You will work closely with families and clinical teams to maintain comfort and dignity during visits.

Ideal candidates are dependable, empathetic, and able to follow care plans precisely, documenting care in our EMR and collaborating with nurses and therapists across multiple shifts.

Formación

• Meets state and federal requirements for Home Health Aide certification or training.
• Ability to read, follow care plans, and document services provided.
• Current CPR certification (where required).

Responsabilidades

• Assist patients with daily living activities such as bathing, dressing, grooming, and toileting.
• Support mobility, transfers, and basic exercises per care plan.
• Maintain a clean, safe home environment.
• Prepare meals and assist with feeding as needed.
• Take and record vital signs when required and trained.
• Document care accurately in the EMR system.
• Report changes in patient condition to the clinical team.
• Participate in team trainings and care coordination activities.
• Provide compassionate support to patients and families during visits.
• Work independently in home-based settings.

What You'll Do

Provide compassionate, hands-on care that supports patient comfort, safety, and independence:

• Assist patients with daily living activities, including bathing, dressing, grooming, and toileting
• Support mobility, transfers, and basic exercises in alignment with the care plan
• Help maintain a clean, safe, and comfortable home environment
• Prepare meals and assist with feeding as needed
• Take and record vital signs when required and trained to do so
• Assist with medication self-administration as directed
• Observe and report changes in patient condition to the clinical team
• Document care provided accurately and timely within the EMR system
• Participate in team meetings, trainings, and care coordination activities
• Provide compassionate support to patients and families during each visit

We're Looking For

We're seeking individuals who are dependable, compassionate, and comfortable working independently in a home-based setting:

• Meets state and federal requirements for Home Health Aide certification or training
• At least 18 years of age
• Ability to read, follow care plans, and document services provided
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
• Ability to work independently with strong time management skills
• Demonstrates empathy, patience, and professionalism in all interactions
