Job summary

VitalCaring, a nationwide home health provider, seeks a compassionate Home Health Aide (HHA) to deliver hands-on care in patients' homes. You’ll support daily living activities, monitor comfort and safety, and document services for the clinical team.

We value dependable, patient-centered individuals who can work independently with strong time management and clear communication. Current HHA certification or training and CPR are preferred; a valid driver’s license and reliable transportation are

Qualifications

• Meets state and federal requirements for Home Health Aide certification or training.
• At least 18 years of age and CPR certification.
• Valid driver’s license with reliable transportation.
• Ability to read care plans, document services, and work independently.

Responsibilities

• Assist patients with daily living activities and personal care.
• Support mobility, transfers, and basic exercises per care plan.
• Prepare meals and assist with feeding as needed.
• Observe changes in condition and report to clinical team.
• Document care accurately within the EMR system.
• Participate in team meetings and care coordination.

What You'll Do

Provide compassionate, hands-on care that supports patient comfort, safety, and independence:

• Assist patients with daily living activities, including bathing, dressing, grooming, and toileting
• Support mobility, transfers, and basic exercises in alignment with the care plan
• Help maintain a clean, safe, and comfortable home environment
• Prepare meals and assist with feeding as needed
• Take and record vital signs when required and trained to do so
• Assist with medication self-administration as directed
• Observe and report changes in patient condition to the clinical team
• Document care provided accurately and timely within the EMR system
• Participate in team meetings, trainings, and care coordination activities
• Provide compassionate support to patients and families during each visit

We're Looking For

We're seeking individuals who are dependable, compassionate, and comfortable working independently in a home-based setting:

• Meets state and federal requirements for Home Health Aide certification or training
• At least 18 years of age
• Ability to read, follow care plans, and document services provided
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
• Ability to work independently with strong time management skills
• Demonstrates empathy, patience, and professionalism in all interactions
