Job summary

VitalCaring is seeking a compassionate Home Health Aide (HHA) to provide hands-on care in patients' homes, helping with daily living activities, safety, and comfort.

You will work independently within care plans, document services in the EMR, and collaborate with nurses and care teams to support patients and families. Strong communication, CPR certification, a valid driver's license, and reliable transportation are required.

Qualifications

• Must meet state and federal Home Health Aide certification or training requirements.
• Be at least 18 years old.
• Ability to read care plans and document services.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Ability to work independently with time management.
• Demonstrates empathy and professionalism.

Responsibilities

• Assist patients with daily living activities (bathing, dressing, grooming, toileting).
• Support mobility, transfers, and exercises per care plan.
• Maintain a clean, safe home environment.
• Prepare meals and assist with feeding.
• Take and record vital signs when required and trained.
• Assist with medication self-administration as directed.
• Observe and report changes to the clinical team.
• Document care accurately in EMR.
• Participate in team trainings and care coordination.
• Provide compassionate support to patients and families during visits.

What You'll Do

Provide compassionate, hands-on care that supports patient comfort, safety, and independence:

• Assist patients with daily living activities, including bathing, dressing, grooming, and toileting
• Support mobility, transfers, and basic exercises in alignment with the care plan
• Help maintain a clean, safe, and comfortable home environment
• Prepare meals and assist with feeding as needed
• Take and record vital signs when required and trained to do so
• Assist with medication self-administration as directed
• Observe and report changes in patient condition to the clinical team
• Document care provided accurately and timely within the EMR system
• Participate in team meetings, trainings, and care coordination activities
• Provide compassionate support to patients and families during each visit

We're Looking For

We're seeking individuals who are dependable, compassionate, and comfortable working independently in a home-based setting:

• Meets state and federal requirements for Home Health Aide certification or training
• At least 18 years of age
• Ability to read, follow care plans, and document services provided
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
• Ability to work independently with strong time management skills
• Demonstrates empathy, patience, and professionalism in all interactions
