Job summary

VitalCaring is seeking a compassionate Home Health Aide to provide hands-on care in patients’ homes. You will assist with daily activities, monitor comfort and safety, and follow care plans under supervision.

Document services in an EMR system and collaborate with nurses and therapists to ensure quality care. A supportive team, flexible scheduling, and growth opportunities are offered by a nationwide home health provider.

Qualifications

• Must have Home Health Aide certification or state training.
• At least 18 years old.
• Ability to read care plans and document services.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Strong time management and independence.
• Empathy and professionalism.

Responsibilities

• Assist patients with daily living activities including bathing, dressing, grooming, and toileting.
• Support mobility, transfers, and basic exercises per care plan.
• Maintain a clean, safe home environment.
• Prepare meals and assist with feeding as needed.
• Take and record vital signs when trained to do so.
• Assist with medication self-administration as directed.
• Report changes to the clinical team and document care in EMR.
• Participate in team meetings and care coordination.

What You’ll Do

Provide compassionate, hands-on care that supports patient comfort, safety, and independence:

• Assist patients with daily living activities, including bathing, dressing, grooming, and toileting
• Support mobility, transfers, and basic exercises in alignment with the care plan
• Help maintain a clean, safe, and comfortable home environment
• Prepare meals and assist with feeding as needed
• Take and record vital signs when required and trained to do so
• Assist with medication self-administration as directed
• Observe and report changes in patient condition to the clinical team
• Document care provided accurately and timely within the EMR system
• Participate in team meetings, trainings, and care coordination activities
• Provide compassionate support to patients and families during each visit

What We’re Looking For

We’re seeking individuals who are dependable, compassionate, and comfortable working independently in a home-based setting:

• Meets state and federal requirements for Home Health Aide certification or training
• At least 18 years of age
• Ability to read, follow care plans, and document services provided
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
• Ability to work independently with strong time management skills
• Demonstrates empathy, patience, and professionalism in all interactions
