Job summary

Custom Home Health is seeking compassionate CNAs/caregivers to join our in-home care team in southeast Michigan. You will provide hands-on assistance with activities of daily living, ensuring comfort, dignity, and safety for patients in their homes.

Responsibilities include personal care, meal support, and adherence to individualized care plans. Must have state certification, CPR current, and at least 1 year of CNA experience. Flexible schedules available to fit your lifestyle.

Responsibilities

What Your Day Looks Like at Custom:

As part of our dedicated team, you’ll provide hands-on personal care to patients, ensuring their comfort, dignity, and well-being by:

• Assisting with personal hygiene needs like bathing, oral care, and grooming.
• Supporting daily activities such as dressing, toileting, and ambulation.
• Maintaining a clean and orderly living environment for patients.
• Preparing and serving nutritious meals, assisting with feeding if needed.
• Following personalized care plans in collaboration with nurses and interdisciplinary teams.
• Observing and reporting significant changes in the patient’s condition to the assigned nurse.
• Documenting care in line with organizational standards and maintaining professional conduct.
• Participating in orientation, staff meetings, and training programs (e.g., QAPI, Emergency Preparedness).

Who You Are:

• Certified Professional: Valid certification through the state (required).
