Qualifications

• Must hold a valid state certification.
• Minimum 1 year of certified nursing experience.

Responsibilities

• Assist with personal hygiene needs like bathing, oral care, and grooming.
• Support daily activities such as dressing, toileting, and ambulation.
• Maintain a clean and orderly living environment for patients.
• Prepare and serve nutritious meals; assist with feeding if needed.
• Follow personalized care plans in collaboration with nurses and interdisciplinary teams.
• Observe and report significant changes in the patient’s condition to the assigned nurse.
• Document care in line with organizational standards and maintain professional conduct.
• Participate in orientation, staff meetings, and training programs (e.g., QAPI, Emergency Preparedness).

What Your Day Looks Like at Custom:
