Adaptive Home Health, LLC is seeking a Home Health Aide to deliver compassionate in-home care in Detroit. You will manage a full caseload, coordinate with the care team, and help patients maintain independence in their homes while building trust with discharge planners and physicians.

This field-based role offers scheduling flexibility, mileage reimbursement, and the chance to contribute to an AI-native platform expanding access to quality home health care.

Qualifications

Must have Michigan HHA certification.
1+ year of experience in personal care.
CPR/BLS certification required.
Valid driver's license and reliable transportation.
Ability to lift and transfer safely.
Strong communication and professional demeanor.
Comfortable working independently in patients' homes.
High school diploma or GED.

Responsibilities

Provide personal care including bathing, dressing, grooming and toileting.
Assist with transfers, ambulation and mobility using proper body mechanics.
Support medication reminders per care plan.
Monitor patient condition and vital signs; report changes to supervising nurse.
Perform light housekeeping to ensure safe home environment.
Document care accurately in HCHB.
Communicate with patients, families and care team for continuity of care.
Follow infection control practices and preserve patient dignity.
Assist with range of motion exercises as ordered.
Coordinate with supervising RN/LVN for care plan updates.

Comp: $20.00-$30.00 /per hour + mileage reimbursement
Schedule: Full-time Monday–Friday + some weekends (as needed)

The mission of this role

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on. The HHA role is essential to that mission: you provide compassionate personal care in the home, support patients in their daily activities, and help us raise the standard for what home health can be.

Objectives

Carry a full, high-quality caseload
Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

What you'll own

Provide personal care assistance including bathing, dressing, grooming, and toileting.
Assist patients with safe transfers, ambulation, and mobility using proper body mechanics.
Support patients with medication reminders as directed by the care plan.
Monitor and report changes in patient condition, vital signs (when ordered), and daily living activities to the supervising nurse.
Perform light housekeeping tasks related to patient safety and care (laundry, meal preparation, maintaining clean environment).
Document all care provided accurately and timely in Homecare Homebase (HCHB).
Communicate clearly with patients, families, and the care team to ensure continuity of care.
Follow infection control practices and maintain patient dignity and privacy.
Assist with range of motion exercises and other restorative care activities as ordered.
Coordinate with supervising RN/LVN for care plan updates and patient needs.

Requirements

Current Michigan HHA certification
1+ years of experience providing personal care (home health, assisted living, nursing home, or hospital setting)
