Custom Home Health Inc. in Troy, MI, is seeking Registered Nurses to join a winning team. This role emphasizes patient-centered care, leadership, and coordination of home health services.

The office offers a hybrid schedule, with hours Monday to Friday, 8:30 am to 5:00 pm. Ideal candidates will have Michigan RN licensure, at least two years in a supervisory capacity in healthcare, and current CPR certification.

Competenze

• Michigan RN license required.
• Minimum 2 years of management or supervisory experience in healthcare (home care preferred).
• Current CPR certification.
• Excellent patient care and leadership abilities.

Mansioni

• Receive and review case referrals.
• Assess patient needs and assign appropriate clinicians.
• Provide guidance and support to ensure quality home care services.
• Collaborate with the team to establish therapeutic goals and develop comprehensive care plans.
• Conduct quarterly record reviews and make recommendations for improvement.
• Assist in screening and interviewing of new organization personnel and make recommendations for employment of individuals.
