Qualifications

• Must be age 18 or older
• Active HHA or CNA certification or willingness to obtain
• Ability to follow written care instructions and document services
• Compassion, patience, and professionalism with vulnerable populations

Responsibilities

• Provide personal care: bathing, grooming, dressing, oral hygiene, toileting
• Assist with mobility, transfers, ambulation, ROM
• Prepare meals and assist with feeding
• Support medication reminders (not administer)
• Report patient condition to supervising nurse
• Record basic vital signs when trained
• Maintain a clean, safe home care environment
• Travel to patient homes per schedule
• Complete required in-service training including dementia education
