Job summary

UnitedHealth Group's Patient Care Hospice of Wayne in Wayne, PA, seeks a Home and Community Based Services Aide to help an individual remain in their own home. This role provides personal care and support with daily living activities, household tasks, and transportation as needed.

The position requires a valid driver’s license, vehicle insurance, reliable transportation, and PA state-specific training or competency. CPR and at least 6 months of home care experience are preferred.

Qualifications

• Current driver's license, vehicle insurance, and reliable transportation or access to public transportation
• PA: competency OR training program meeting 28 Pa. Code § 611.55 OR a home health aide training program meeting 42 CFR 484.36 OR agency training standards for Medicaid Waiver/PCS

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
