Job summary

Pathways Healthcare, Llc is seeking a Hospice Home Health Aide to support patients in Worcester and surrounding areas. The role offers a Monday–Friday schedule from 8:30 AM to 5:00 PM and an hourly rate of $25.00.

Qualified CNAs are preferred in Massachusetts; Home Health Aide certification is accepted. Candidates must be 18+, with CPR, a valid driver's license, and the ability to follow care plans and document services provided.

Qualifications

• Must be 18 years of age or older.
• CNA preferred; Home Health Aide certification accepted.
• Ability to read and follow written care plans and document services provided.
• Ability to work independently with minimal supervision.
• Current CPR certification required.
• Strong interpersonal skills and professionalism.
• Valid driver’s license and reliable insured vehicle.
• Ability to assist patients with repositioning, transferring, and other physical tasks.

Responsibilities

• Provide personal care assistance such as bathing, grooming, dressing, and mobility support.
• Observe and report changes in patients’ condition to the supervising nurse or therapist.
• Document care provided and patient observations accurately.
• Follow the care plan established by the Registered Nurse.
• Maintain a safe and supportive environment for patients in their homes.
• Promote quality, continuity, and safety of patient care.
