Job summary

Pathways Healthcare, Llc in Massachusetts is seeking a Home Health Aide (CNA) to join our physician- and NP-led home health and hospice team, providing personal care in patients' homes, under supervision of RN or therapist.

You will observe and document patient care, maintain quality and safety, and collaborate with caregivers; position is full-time, Monday-Friday, with benefits including 401(k) match and health plans; hourly rate $25.

Qualifications

• Certified Nursing Assistant license preferred in MA or Home Health Aide certificate accepted
• At least 18 years of age
• Ability to read and follow written instructions and document care
• Self-directed with ability to work with little supervision
• Empathy for the needs of the ill, injured, frail and the impaired
• Possess and maintain CPR certification
• Licensed driver with insured vehicle in good working order

Responsibilities

• The Home Health Aide is a CNA/paraprofessional who works under supervision of a registered nurse or therapist and performs personal care services as necessary to meet patient needs.
• HHA observes patients, reports observations, and documents care performed.
• HHA is assigned by a Registered Nurse to promote quality, continuity and safety of care.

Responsibilitie s:

• The Home Health Aide is a Certified Nursing Assistant and paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient’s needs. The HHA is responsible for observing patients, reporting these observations and documenting observations and care performed. The HHA will be assigned by a Registered Nurse in a manner that promotes quality, continuity and safety of a patient’s care.

Qualifications:

1. Certified Nursing Assistant in Massachusetts (preferred); Home Health Aide certificate accepted

2. At least 18 years of age

3. Ability to read and follow written instructions and document care given

4. Self-directing with the ability to work with little direct supervision

5. Empathy for the needs of the ill, injured, frail and the impaired

6. Possess and maintains current CPR certification

7. Licensed driver with automobile that is insured in accordance with Organization requirements and is in good working order
