Job summary

Pathways Healthcare is a leading home health and hospice organization dedicated to patient-centered care. The Home Health Aide (HHA) / Licensed Nursing Assistant (LNA) supports patients with daily activities, monitors health, and communicates observations to the clinical team under supervision of a Registered Nurse or therapist.

This full-time role in Londonderry, NH offers a Monday through Friday schedule and an hourly rate of $25.00, with opportunities for growth and professional development

Qualifications

  • Licensed Nursing Assistant (CNA) in NH preferred; Home Health Aide certification accepted.
  • Must be 18 years of age or older.
  • Ability to read and follow written care plans and document services provided.
  • Ability to work independently with minimal supervision.
  • Compassion and empathy for individuals who are ill, injured, or elderly.
  • Current CPR certification required.
  • Strong interpersonal skills, patience, and professionalism.
  • Valid driver’s license and reliable insured vehicle.
  • Ability to assist patients with repositioning, transferring, and other physical tasks.

Responsibilities

  • Provide personal care assistance such as bathing, grooming, dressing, and mobility support.
  • Observe and report changes in patients’ condition to the supervising nurse or therapist.
  • Document care provided and patient observations accurately.
  • Follow the care plan established by the Registered Nurse.
  • Maintain a safe and supportive environment for patients in their homes.
  • Promote quality, continuity, and safety of patient care.

Position Overview

The Home Health Aide (HHA) / Licensed Nursing Assistant (LNA) is a paraprofessional member of the home care team who provides personal care services to patients under the supervision of a Registered Nurse or therapist.

In this role, you will support patients with daily activities, monitor their condition, and communicate observations to the clinical team to ensure safe, high-quality care.
