Job summary

Pathways Healthcare, a home health and hospice provider, is seeking compassionate CNAs to join our in-home care team serving Concord, MA and surrounding areas. Per diem and part-time shifts are available, with an hourly rate of $25.

You will assist with activities of daily living, observe patients, document care, and follow care plans under supervision, ensuring a safe and dignified healing environment for clients at home.

Qualifications

• Certified Nursing Assistant (CNA) in Massachusetts preferred; Home Health Aide certification accepted.
• Ability to read and follow written care plans and document services provided.
• Ability to work independently with minimal supervision.
• Compassion and empathy for individuals who are ill, injured, or elderly.
• Current CPR certification.
• Strong communication and interpersonal skills.
• Valid driver’s license and reliable, insured vehicle.

Responsibilities

• Provide personal care assistance such as bathing, grooming, dressing, and mobility support
• Observe and report changes in patients’ condition to the supervising nurse or therapist
• Document care provided and patient observations accurately
• Follow the care plan established by the Registered Nurse
• Maintain a safe and supportive environment for patients in their homes
• Promote quality, continuity, and safety of patient care
