Job summary

Pathways Healthcare is a leading home health and hospice provider led by physicians and nurse practitioners. We are seeking a dedicated CNA to join our in-home care team in Concord, MA, to support patients with daily activities and report changes to the supervising nurse.

This role offers a flexible schedule, hourly pay of $25, opportunities for growth and ongoing professional development, and a collaborative, patient-centered culture.

Qualifications

• CNA (Massachusetts) preferred; Home Health Aide accepted.
• Ability to read and follow care plans and document services provided.
• Strong communication and interpersonal skills.
• Current CPR certification and valid driver’s license.

Responsibilities

• Provide personal care assistance (bathing, grooming, dressing, mobility).
• Observe and report changes to supervising nurse or therapist.
• Document care provided and patient observations accurately.
• Follow the care plan established by the Registered Nurse.
• Maintain a safe and supportive home environment and promote quality of care.

Responsibilities

• Provide personal care assistance such as bathing, grooming, dressing, and mobility support
• Observe and report changes in patients’ condition to the supervising nurse or therapist
• Document care provided and patient observations accurately
• Follow the care plan established by the Registered Nurse
• Maintain a safe and supportive environment for patients in their homes
• Promote quality, continuity, and safety of patient care

Qualifications

• Certified Nursing Assistant (CNA) in Massachusetts preferred; Home Health Aide certification accepted
• Ability to read and follow written care plans and document services provided
• Ability to work independently with minimal supervision
• Compassion and empathy for individuals who are ill, injured, or elderly
• Current CPR certification
• Strong communication and interpersonal skills
• Valid driver’s license and reliable, insured vehicle
