Emblem Home Health & Hospice, a The Pennant Group, Inc. company, seeks an RN Admit for weekend coverage serving West Valley locations including Sun City, Surprise, Goodyear and Litchfield Park.

You will perform Start of Care visits, assess home safety, and plan patient-specific care in the home setting. The role emphasizes wound and infusion care, case coordination with the care team, and thorough OASIS documentation.

Qualifikationen

• Licensed registered nurse in Arizona or able to obtain in-state license.
• Minimum 1 year Home Health nursing preferred.
• Wound care and/or infusion experience preferred.
• Strong clinical skills and patient-centered approach.
• Excellent organizational and time-management abilities.
• Current CPR certification.
• Valid AZ driver’s license, vehicle insurance, and reliable transportation.

• Conduct wound assessments and provide wound care as needed.
