Emblem Home Health in Phoenix, AZ is seeking a Licensed Practical Nurse (LPN) to provide hands-on patient care in patients' homes across Phoenix, Avondale, Glendale, Buckeye, and Peoria. The role requires an AZ LPN/LVN license, CPR, and a valid driver’s license, with travel across the West/Central area.

You will implement care plans, monitor patient conditions, and collaborate with RNs and the care team to ensure high-quality service.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current LPN/LVN license in Arizona.
• Minimum 3 years nursing experience preferred (Home Health or Med/Surg is a plus).
• Current CPR certification.
• Valid driver's license, auto insurance, and reliable transportation.

Responsibilities
