Job summary

Emblem Home Health & Hospice in Goodyear, AZ is hiring a Home Health Admit RN to deliver high-quality nursing care in patients' homes across the West Valley, including Sun City, Sun City West, El Mirage, Surprise, Litchfield Park and Goodyear. This full-time role requires Saturday and Sunday coverage.

You will lead SOC visits, coordinate interdisciplinary care, perform wound care and IV/infusion therapies, and document OASIS accurately while empowering patients and families to manage their care

Qualifications

• Currently licensed as a Registered Nurse (RN) in AZ.
• Minimum of 1 year Home Health preferred.
• Wound care and/or infusion experience preferred.
• Strong clinical and caregiving skills.
• Excellent organization and time management.
• Current CPR certification.
• Valid AZ driver's license and auto insurance.

Responsibilities

• Perform Start of Care (SOC) visits and comprehensive assessments.
• Coordinate care with physicians, therapists, and the team.
• Provide wound assessments and wound care when needed.
• Administer IV/infusion therapies as ordered.
• Complete accurate OASIS documentation.
• Educate patients and caregivers to promote independence.

Y** our Impact**

As a Home Health Admit RN , you’ll be the heart of patient care, bringing clinical expertise and compassion directly into the home. This role combines Start of Care (SOC), wound care, and infusion therapy responsibilities, providing high-quality nursing care to patients in their homes. You will:

• Perform Start of Care (SOC) patient visits, including initial physical assessment, medication reconciliation, home safety evaluations
• Create personalized care plans to improve patient outcomes and support their independence
• Perform wound assessments and provide wound care treatments if necessary
• Administer and monitor IV/infusion therapies per physician orders when required
• Complete accurate and timely clinical OASIS documentation
• Act as a trusted Case Manager, coordinating care with physicians, therapists, and the interdisciplinary team
• Educate and empower patients and caregivers with confidence

Our Requirements

• Minimum of 1 year Home Health preferred
