Job summary

Emblem Home Health in Phoenix, AZ is seeking a part-time Home Health Admit RN for Saturdays and Sundays. You will perform Start of Care visits, assess patients, reconcile medications, and develop personalized care plans to support independence.

You will deliver wound care, administer IV therapies when ordered, document OASIS, coordinate with physicians and therapists, and educate patients and caregivers. Arizona RN license and CPR certification are required, along with a valid driver's license.

Qualifications

• Arizona RN license is required for practice in the state.
• CPR certification is required.
• Wound care and/or infusion therapy experience preferred.

Responsibilities

• Perform Start of Care visits with initial assessment and medication reconciliation.
• Create personalized care plans to improve outcomes and independence.
• Conduct wound assessments and provide wound care as needed.
• Administer and monitor IV/infusion therapies per physician orders.
• Complete timely clinical OASIS documentation.
• Act as Case Manager coordinating care with physicians and therapists.
• Educate patients and caregivers to build confidence in care.
