Job summary

Connected Home Health in the Portland metro area is seeking a full-time Admissions Registered Nurse to perform start-of-care assessments, initiate care plans, and coordinate home-health services for our local patients.

This daytime, field-based role offers predictable scheduling, strong interdisciplinary support, mileage reimbursement, and growth opportunities within The Pennant Group. Oregon RN license with about one year of nursing experience preferred (home health or admissions a plus).

Responsibilities

• Complete SOC nursing assessments in patient homes.

Position Overview Admissions RN Connected HH is seeking a full‑time, hourly Home Health Admissions RN to support our Portland Metropolitan service area. This role is ideal for nurses who excel in first‑visit assessments, care‑plan initiation, and establishing a strong foundation for each patient’s home‑health episode. This position follows standard business hours, Monday through Friday, providing predictable scheduling, strong interdisciplinary support, and a balanced workflow.

Key Responsibilities Admissions RN Responsibilities

• Complete comprehensive Start‑of‑Care (SOC) nursing assessments in patient homes.
• Initiate individualized care plans and establish expectations for the home‑health episode.

• Communicate effectively with physicians, caregivers, and internal team members.
• Support safe transitions from hospitals, SNFs, and outpatient settings.

Schedule Business Hours Monday–Friday Normal business hours (daytime schedule) No evenings, nights, or rotating weekends Predictable workflow with full team availability during working hours

Qualifications RN Requirements

• Current Oregon RN license (or ability to obtain).
• One year of nursing experience preferred; home‑health or admissions experience a plus.
• Strong assessment, communication, and critical‑thinking skills.
• Ability to work independently in a field‑based role.
• Reliable transportation and valid driver’s license.
