Connected Home Health in the Portland metropolitan area is seeking a full-time, hourly Admissions RN to support our local service area. You will perform Start-of-Care assessments in patients' homes, initiate individualized care plans, and lay a solid foundation for each home‑health episode.

This role offers predictable Monday–Friday hours, strong interdisciplinary support, and opportunities for professional growth within The Pennant Group’s Portland Metropolitan area network.

Qualifications

• Current Oregon RN license (or ability to obtain).
• One year of nursing experience preferred; home-health or admissions experience a plus.
• Strong assessment, communication, and critical-thinking skills.
• Ability to work independently in a field-based role.
• Reliable transportation and valid driver’s license.

Responsibilities

• Complete comprehensive Start-of-Care (SOC) nursing assessments in patient homes.
• Initiate individualized care plans and establish expectations for the home-health episode.
• Conduct medication reconciliation, safety assessments, and disease-specific education.
• Coordinate care with interdisciplinary team members to ensure timely service initiation.
• Provide skilled nursing interventions as needed during SOC visits.
• Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
• Communicate effectively with physicians, caregivers, and internal team members.
• Support safe transitions from hospitals, SNFs, and outpatient settings.

Position Overview Admissions RN Connected HH is seeking a full‑time, hourly Home Health Admissions RN to support our Portland Metropolitan service area. This role is ideal for nurses who excel in first‑visit assessments, care‑plan initiation, and establishing a strong foundation for each patient’s home‑health episode. This position follows standard business hours, Monday through Friday, providing predictable scheduling, strong interdisciplinary support, and a balanced workflow.
