Job summary

Adara Home Health in Minnesota seeks a Clinical Admissions Nurse for full-time, 1:1 in-home care across Hutchinson, Waconia and surrounding cities, Monday–Friday with on-call rotation.

You will perform initial and periodic client assessments, develop and update care plans, administer treatments as ordered, and document outcomes and nursing notes per policy, reporting to the Clinical Supervisor.

Qualifications

• Current RN license and ability to practice in Minnesota.
• Experience in home health or community nursing preferred.
• Strong assessment and documentation skills.

Responsibilities

• Perform initial and periodic client assessments and update care plans.
• Provide total client care including medications and treatments as ordered.
• Document diagnoses, outcomes and interventions per company standards.
• Maintain up-to-date care plans as client condition changes.

The Clinical Admissions Nurse ensures client care is provided in accordance with the company Policies and Procedures. Maintains strong relationships with referral sources. The position reports to the Clinical Supervisor.

SCHEDULE

• This is a full time position, Monday - Friday with an on-call rotation.
• This position will require you to travel to client's homes to provide 1:1 client care that may take 1-3 hours per client.

COVERAGE

• This position reports to the Hutchinson, MN office and provides services in Waconia and surrounding cities.

MAJOR RESPONSIBILITIES

• Client Care:
  • Performs initial and periodic assessment of clients to determine needs. Implements and evaluates the effectiveness of care.
  • Uses interview and physical assessment skills to evaluate client needs.
  • Demonstrates nursing skills appropriate for services provided.
  • Records nursing diagnoses, measurable outcomes, interventions and nurses notes according to company standards.
  • Develops and updates care plans as client's condition indicates.
• Demonstrates knowledge of current health practices when performing observation and evaluation of client care.
• Provides total client care to include medications and treatments administered as ordered by the physician, physical and psychological care,
