Job summary

Adara Home Health in Minnesota is seeking a Licensed Practical Nurse to provide 1:1 client care under the supervision of a Clinical Supervisor. The role requires current LPN licensure in Minnesota and at least one year of health care experience.

The position is part-time with a flexible schedule, including evening and weekend shifts. You will travel to client homes in Janesville and surrounding areas to deliver skilled nursing services per the care plan.

Qualifications

• Must hold Minnesota LPN license in good standing.
• Minimum 1 year healthcare experience; hospital or home care preferred.
• Valid driver's license and access to reliable transportation.
• CPR certification highly recommended.
• Ability to work independently and as part of an interdisciplinary team.

Responsibilities

• Plan, provide and evaluate nursing care per policies and case plan.
• Recognize symptoms and notify Case Manager of changes immediately.
• Document observations accurately and timely.
• Administer medications and treatments as ordered within legal limits.
• Engage in ongoing professional development and education.
• Maintain client safety and confidentiality at all times.
• Participate in after-hours on-call process as needed.
