Job summary

Graham Healthcare Group is seeking an Admission Registered Nurse (RN) for Home Health in Bunola, Pennsylvania. This role delivers skilled, patient-centered care in patients' homes and involves start-of-care assessments, care planning, and coordinating with an interdisciplinary team.

You'll work independently in the field with about 20% travel to patient locations and will be eligible for a $10,000 first-year success bonus, plus a comprehensive benefits package and ample opportunities for growth.

Qualifications

• Active Registered Nurse license in the state you work
• One year of RN experience
• Valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Conduct comprehensive start-of-care assessments, including clinical evaluations, medication reconciliation, and development of initial care plans aligned with physician orders
• Educate and guide patients and families through the admission process, setting expectations for care, reviewing diagnoses and treatment plans, and ensuring they feel informed and supported from day one
• Apply strong clinical judgment and critical thinking to coordinate admissions efficiently, address barriers to care, and ensure accurate, timely, and compliant EMR documentation
• Initiate evaluations to other disciplines when indicated
• Work independently, but never alone
• Travel to patient homes within assigned territory
