Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) - Admission for a Home Health position in New Eagle, PA. This role supports skilled nursing care in patients' homes and includes start-of-care assessments and care-plan development.

You will travel to patient locations within the territory, collaborate with an interdisciplinary team, and educate families about the admission process. A valid RN license and driving are required; first-year bonus opportunities are available.

Qualifications

• Active RN license in the state you work.
• Minimum of 1 year RN experience in home health or similar.
• Valid driver’s license; ability to spend ~20% of your day driving to patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting.
• Conduct start-of-care assessments and develop initial care plans.
• Educate patients and families through the admission process.
• Apply clinical judgment to coordinate admissions and EMR documentation.
• Initiate evaluations to other disciplines when indicated.
• Travel to patient homes within assigned territory.
