Qualifications

• Active RN license in the state you work.
• One year of RN experience and the clinical expertise to deliver quality patient care.
• Valid driver’s license and the ability to spend ~20% of workday driving to patient locations.

Responsibilities

• Deliver skilled, patient-centered home health care in collaboration with an interdisciplinary team.
• Conduct comprehensive start-of-care assessments, including clinical evaluations, medication reconciliation, and initial care plan development.
• Educate and guide patients and families through the admission process, setting expectations for care and reviews of diagnoses.
