Graham Healthcare Group is hiring an Admission Registered Nurse (RN) to provide skilled nursing care to patients in their homes in Monongahela, PA. The role offers autonomy, meaningful patient relationships, and opportunities for growth within a national home health and hospice organization.

The RN will conduct starts of care, coordinate through an interdisciplinary team, and educate families about care plans while navigating the admission process and ensuring compliant documentation.

Qualifications

• Active RN license in the state you work
• One year of RN experience delivering skilled care
• Valid driver’s license and ability to drive 20% of the day

Responsibilities

• Deliver skilled, patient-centered care in home health settings with interdisciplinary support
• Conduct start-of-care assessments, med reconciliation, and initial care plan development
• Educate patients and families about admission process, diagnoses, and care expectations
• Apply clinical judgment to coordinate admissions and ensure compliant EMR documentation
• Initiate evaluations to other disciplines when indicated
• Travel to patient homes within assigned territory
