Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team
• Conduct start-of-care assessments, clinical evaluations, medication reconciliation, and initial care plans
• Educate patients and families through the admission process and set expectations for care
• Apply clinical judgment to coordinate admissions, address barriers, and ensure accurate EMR documentation
• Initiate evaluations to other disciplines when indicated
• Work independently and travel to patient homes within territory
