Description

Summary

This position has overall responsibility of providing leadership, direction, and administration of day-to day operations associated with direct patient care activities within the Chickasaw Nation Department of Health Home Health services This position is responsible for driving, supporting and modeling a service- oriented culture focused on employee engagement, quality, patient safety, service excellence, fiscal responsibility, and the overall patient experience. Develops, directs the implementation and ensures compliance of standards of practice that promote optimum health care delivery. Serve as a member of the leadership team at the facility and system levels, building and supporting effective collegial relationships with applicable internal and external constituents and stakeholders and ensuring optimal operating effectiveness and strategic positioning. The position may require scheduled duty on days, evenings, nights, weekends, holidays and call backs.

Responsibilities
  • Maintains a working knowledge of current clinical scope of practice and the regulatory requirements affecting that practice and exhibits the value of continuous learning
  • Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards
  • Leads staff to provide all customers with an excellent service experience by consistently demonstrating our core behaviors
  • Assures Joint Commission and Centers for Medicare and Medicaid Services (CSM) requirements are being met consistently, and staff are practicing with continued readiness
  • Establishes and enhances a culture of evidence-based decision making in clinical and management initiatives
  • Participates in departmental performance improvement activities to measure, assess, and improve the quality of nursing and patient care delivered to patients
  • Understands utilization management and case management and their importance to the organization
  • Works with risk management to develop strategies that effectively manage risk
  • Strategizes and drives process improvements focused on innovative care delivery and/or operational models designed to improve clinical service supports, outcomes, patient throughput, and patient safety
  • Evaluates and promotes the use and implementation of technology in the workplace in order to streamline operations, facilitate communications, and optimize work processes
  • Ensures and fosters a high level of collaboration within a highly complex team environment in order to coordinate activities, review work, exchange information, and resolve problems
  • Functions as an integral member of the hospital operations and system leadership teams, advisory capacity to internal peers as well as other divisions within the Chickasaw Nation
  • Promotes financial stewardship and clinical excellence for the facility in both operational and capital acquisition processes
  • Allocates financial information and human capital for improvement activities, ensuring the efficient delivery of cost-effective services to patients, physicians, and hospital departments
  • Establishes performance measures, assesses, and evaluates divisional operations, and works with department leadership to assure efficient and effective delivery of services
  • Analyzes and implements cost containment measures and performance indicators to ensure responsible stewardship of all assigned budgets and maximum effectiveness of funds expended
  • Participates in patient grievance process as needed
  • Leads the department to meet objectives of strategic plan and performance improvement initiatives
  • Actively involved in various committees, i.e. Governing Body, Medical Staff, Department Head, and Leadership Council
  • Develops and participates in programs of education and training; including orientation of new employees, continuing education of existing staff and ongoing program development for workforce development
  • Facilitates the assessment and modification of personnel management policies to ensure reasonable hours and acceptable working conditions to provide staff coverage
  • Ensures program is compliant and operates within approved budget
  • Gathers, prepares, directs routine narrative and statistical information submitted to the Executive Offer, Ancillary and Specialty Services
  • Develops and maintains documentation as appropriate -- files, logs, records, reports, etc.
  • Maintains knowledge of, follows, and enforces all rules, regulations, policies and guidelines for assigned area
  • Meets goals by establishing productivity, quality and service standards, and identifies improvement opportunities
  • Performs all other duties as assigned

  • Must possess current Oklahoma Home Care Administrator certificate that complies with provisions of the Oklahoma Statutes, Title 63 Section 1-1962 et seq. and the Home CME Administrator Rules and authorized to operate, manage, supervise, or be in charge of a licensed home care agency
