• Meet with referral sources to educate them about agency services.
• Educate referral sources on documentation and admission criteria for home care.
• Collaborate with leadership to plan engagement with payers and sources.
• Implement strategies to expand accounts through regular outreach.
• Provide guidance to referral sources and staff to ensure service alignment.
