Job summary

Pruitthealth Corporation in Valdosta, GA is seeking a Care Manager Home First to oversee admissions, coordinate services with providers, and drive efficient referral workflows. You will conduct in-home assessments, manage physician and community partner relationships, and ensure accurate documentation and tracking of care steps.

You will travel to member homes as needed, participate in trainings, and support on-call rotations to ensure continuity of care and high-quality service.

Qualifications

• Must have a valid driver’s license and automobile insurance liability.
• Graduate of a state approved accredited nursing school.
• Certified in Basic Cardiopulmonary Resuscitation (CPR) is required.
• Hospice or SOURCE care experience preferred.

Responsibilities

• Screens referrals and coordinates admission process.
• Coordinates services with providers and handles billing reconciliation.
• Provides outreach to community referral sources.
• Markets area for awareness of SOURCE and PRN meetings with new PCPs.
• Completes new admission paperwork and in-home assessments.
• Participates in weekly team meetings and presents new admits and discharges.
• Performs quarterly home and PCP visits with required documentation.
• Tracks hospital/ER visits and coordinates with hospitals for records.
• Travel using personal vehicle; arranges transportation as needed.
• Attends trainings and participates in on-call rotations.
