Satellite Healthcare is seeking a Home Therapy RN to coordinate patient care, educate patients, and advocate for informed decisions. The role includes home visits to evaluate home dialysis suitability and collaboration with a multidisciplinary team to deliver exceptional care.

The position requires nephrology dialysis experience, an active RN license, CPR certification, and a valid driver's license for home visits. This full-time role offers comprehensive benefits and growth opportunities.

Formación

• One year of registered nurse experience.
• One year of nephrology nurse with dialysis experience (peritoneal dialysis, home hemodialysis, or both).
• Graduation from an accredited nursing school or equivalent.
• Current state RN licensure.
• Current CPR certification.
• Knowledge of Electronic Health Records.
• Valid state driver''s license and reliable transportation for home visits.

Responsabilidades

• Initiate the practice of a SHC coordinator of patient care, collaborating with other care providers and health team members.
• Act as a patient teacher and advocate for informed consent and patient independence.
• Perform home visits to assess home dialysis suitability in compliance with CMS regulations.
• Collaborate with multi-disciplinary team to deliver high-quality patient care.
