Satellite Healthcare seeks a Home Therapy RN to coordinate patient care with a multidisciplinary team. You will perform home visits to confirm the home is safe for dialysis and in compliance with CMS regulations, and you will educate patients and act as their advocate.

You will promote informed consent, patient independence, and ongoing professional development through education, quality improvement, and research application.

Qualifications

• One year of registered nurse experience.
• One year of nephrology dialysis experience required (PD or home HD).
• Graduation from an accredited nursing school.

Responsibilities

• Provide home-based nursing care and assess home environment for safe at-home dialysis.
• Educate patients, advocate for informed consent, and promote independence.
• Collaborate with a multidisciplinary team and utilize CMS guidelines.
