Job summary

Satellite Healthcare seeks a Home Therapy RN in Oakland, CA to lead home dialysis initiatives and coordinate care with a multi-disciplinary team. You will educate patients, advocate for informed consent, and promote independence while maintaining high standards of safety and quality.

Responsibilities include visiting patient homes to assess suitability for home dialysis under CMS regulations, pursuing ongoing professional development, and applying evidence-based practices to improve patient

Qualifications

• One year of registered nurse experience.
• One year of experience as a Nephrology Nurse with dialysis experience required; in peritoneal dialysis, home hemodialysis, or a combination of both.
• Graduation from an accredited nursing school or equivalent
• Current, active, unencumbered state RN licensure
• Current Healthcare Provider certification (CPR) required

Responsibilities

• The RN identifies professional role development activities including continuing education, quality assessment and improvement, and the review and clinical application of research findings.
• Will perform home visits to determine that the home environment is suitable for patient dialyzing at home and whether or not the home will be a safe and appropriate setting in compliance with CMS regulations.
