Qualifications

• Valid unencumbered state RN licensure.

Responsibilities

• Learn SHC coordination of patient care and collaborate with the care team.
• Act as patient teacher and advocate to support informed decisions.
• Promote patient independence and continuing education.
• Perform home visits to assess dialyzing at home suitability and CMS compliance.
