Job summary

Fresenius Medical Care is seeking an entry level Home Therapies RN CAP to coordinate and provide care for home dialysis patients. The role involves assessment, planning, intervention, and evaluation per established protocols under supervision of a Nurse Educator or Lead Nurse.

The position supports education for patients and families, participates in quality improvement, and may require travel to patient homes or training sites as needed.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful completion of CPR BLS Certification.
• Must meet the practice requirements in the state in which he or she is employed.

Responsibilities

• Provide and coordinate clinically competent care for an assigned group of patients.
• Assess, plan, implement and evaluate care for home dialysis patients per protocols.
• Educate patients and families about home dialysis training and treatment.
• Administer medications as prescribed and document effectiveness.
• Collaborate with interdisciplinary team for patient care and quality improvement.
