Job summary

Herewith is seeking a compassionate Care Partner in Arlington, TX to support a patient transitioning to home dialysis. You will provide one-on-one care, set up dialysis treatment, maintain a clean and safe environment, and offer companionship to help the patient stay on schedule.

Training will be provided by our clinical team to cover patient-specific dialysis processes, home setup, and ongoing guidance, ensuring you feel confident in this meaningful, independent role within a dedicated care

Qualifications

• 1+ year of dialysis experience (home or clinic).
• Open to in-home dialysis support and willing to complete the provided training.
• Familiarity with home dialysis systems preferred (NxStage, Tablo is a plus).
• Steady, supportive, and patient—especially during an adjustment period.
• Strong communicator who works well with a clinical team.
• Must pass a Herewith background check.

Responsibilities

• Assist with dialysis treatment setup (NxStage or Tablo).
• Keep the treatment environment clean, organized, and safe.
• Offer companionship and emotional reassurance.
• Help the patient remain on schedule with treatment times.
• Provide light ADL support as needed.
• Monitor safety and report concerns to the coordinating nurse.
• Maintain simple documentation and shift notes.
