Job summary

Jefferson County is seeking a part-time Home Delivered Meal Assessor. The role evaluates client needs to assure nutrition resources are available, with ongoing in-home assessments and client education as a core focus.

Responsibilities include interviewing participants, developing nutrition education materials, covering meal site managers as needed, attending quarterly meetings and annual training, and complying with the Wisconsin Food Code.

Qualifications

• 1–2 years of experience or training; or a high school diploma or equivalent.
• Serve safe certification required within 90 days of hire.
• Valid driver's license required.

Responsibilities

• Interviews potential meal program participants and completes in-home assessment.
• Assists with developing nutrition education materials and presentations.
• Provides coverage for meal site managers as needed.
• Participates in quarterly site manager meetings and annual training.
• Adheres to Wisconsin Food Code and food safety regulations.
