Job summary

Greater Nashua Mental Health Center is seeking a Community Support Specialist to provide community-based support to individuals on their recovery journey. You will engage clients in their homes and communities using strengths-based methods and evidence-based practices to promote independence and functioning in natural environments.

Qualifications

• Associate's degree in Psychology or related field required.
• 2+ years of experience in human services or behavioral health.
• Knowledge of mental health conditions and recovery-oriented practices.
• Strong communication, interpersonal, and engagement skills.
• Proficiency with Microsoft Office and electronic documentation systems.

Responsibilities

• Provide direct, community-based support services to individuals in their homes and community settings.
• Engage clients using evidence-based interventions such as Motivational Interviewing.
• Maintain a caseload and meet productivity expectations for direct client service hours.
• Conduct outreach to re-engage clients who have disengaged from treatment.
• Complete accurate and timely documentation for all services and outreach activities.
• Collaborate with multidisciplinary teams to support coordinated, client-centered care.
• Participate in team meetings, supervision, and ongoing training activities.
• Maintain schedules and document services in the EMR.
• Support clients in building life skills, accessing resources, and achieving recovery goals.
• Complete mileage and expense reporting in a timely manner.
• Perform additional duties or special projects as assigned.
