Qualifications

• Associate's degree in Psychology or related field required; Bachelor's preferred.
• 2+ years in human services or behavioral health.
• Strong communication and engagement with clients; ability to work independently.
• Proficiency with MS Office and EMR documentation.

Responsibilities

• Provide direct, community-based support in homes and community settings.
• Engage clients using Motivational Interviewing.
• Maintain a caseload and meet productivity expectations for direct client service hours.
• Complete accurate documentation for services and outreach.
• Collaborate with multidisciplinary teams to support coordinated, client-centered care.
• Participate in team meetings, supervision, and ongoing training.
