Job summary

Rehab Without Walls Neuro Rehabilitation in the San Jose area seeks an experienced Physical Therapist to deliver home- and community-based rehabilitation. You will evaluate patients, design and adjust treatment plans, and support independence in daily tasks like cooking, shopping, and community navigation.

The role offers a flexible schedule, opportunities for per diem coverage, mileage reimbursement, and ongoing education programs to support clinical excellence and career growth.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted PT license in the practicing state.
• CPR/BLS certification as required by state regulations.
• Ability to lift up to 50 pounds and handle patient transfers.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess effectiveness and adjust plans.
• Discharge evaluations and provide home/community safety recommendations.
• Communicate patient needs and progress to the care team and family.
• Collaborate with other disciplines to optimize outcomes.
