Job summary

Elara Caring is seeking a Care Team Supervisor for Personal Care Services in Des Plaines, IL. This full-time, on-site role from Monday to Friday, 8a–5p, offers a salary range of $22–$24 per hour.

You will lead a team of caregivers, coordinate services, ensure compliance, and drive branch growth while delivering compassionate home care. You will report to the Care Team Manager or Regional Director and collaborate with payroll, timekeeping, and leadership across the organization.

Qualifications

• High School Diploma or GED required.
• Associate or bachelor’s degree preferred.
• Five (5) years of experience in a fast-paced office or healthcare environment preferred.
• Working knowledge of Medicare/Medicaid and home care operations preferred.
• Strong computer proficiency and documentation skills.
• Excellent organizational and multitasking abilities.
• Professional verbal and written communication skills.
• Strong leadership, coaching, and motivational skills.
• Ability to thrive in a fast-paced, high-volume environment.
• Bilingual fluency may be required based on location.
• Reliable transportation and ability to travel locally as needed.

Responsibilities

• Coordinate and schedule personal care services.
• Maintain client service records, plans of care, caregiver schedules, and documentation.
• Review and manage service authorizations and contract-specific requirements.
• Monitor billing, payroll, utilization, and service delivery reports.
• Support branch growth initiatives and operational planning.
• Ensure compliance with licensure, payer, contract, and regulatory standards.
• Provide guidance, coaching, and support to caregivers.
• Ensure caregiver trainings and certifications are up to date.
• Conduct caregiver orientations as required.
• Collaborate with payroll and timekeeping teams.
• Monitor daily service delivery and caregiver coverage.
• Monitor authorizations vs. scheduled services and review utilization reports.
• Communicate professionally with caregivers, clients, case managers, and leadership.
• Represent Elara Caring in the community.
• Perform additional duties to support team and organizational success.
