Job summary

Sacred Heart Mission invites applications for a casual Support Worker Home Care to help clients in their own homes maintain independence and improve quality of life across the Northern Metro area.

The role requires empathy and resilience to support people from diverse backgrounds and to respond appropriately to various needs. You will perform personal care, daily tasks, and accompany clients to appointments while documenting progress notes.

Qualifications

• Certificate III in Aged Care or Individual Support (Ageing).
• A current Driver's License, a reliable registered car, with comprehensive motor vehicle insurance.
• Experience working within the aged care or support services sector.
• Ability to adapt to clients' needs effectively and while inclusive of client voice.
• Good interpersonal skills, communication and willingness to address and work through challenges of clients.

Responsibilities

• Supporting clients to be independent.
• Maintaining a clean and safe home environment for clients.
• Providing meaningful social interaction.
• Assisting with personal care, shopping, travel to appointments or daily activities.
• Supporting clients to meet care needs and attend medical appointments.
• Completing progress notes for each client at the end of each shift.
• Escalating care needs to Coordinators and Case Managers.
