The Skilled Weekend RN Case Manager contributes to our Mission of Caring by providing compassionate direct patient care to our clients in a one-on-one setting in their homes throughout Rhode Island. The Skilled Weekend RN Case Manager conducts skilled nursing visits to include admissions, revisits, and discharges while providing assessments and education to patients. The Skilled Weekend RN Case Manager for Saint Elizabeth Home Care is a part of a collaborative interdisciplinary team with ample administrative support. Our Skilled Weekend RN Case Manager is equipped with point of care devices (iPad), as well as all necessary and requested medical equipment and supplies.

Qualifications for a Skilled Weekend RN Case Manager:

• Current unencumbered RN license in Rhode Island.
• Minimum of one year nursing experience, home care experience preferred.
• Willingness and ability to drive to patient homes throughout Rhode Island.
• CPR certification.
• Reliable transportation, current clean driver's license, automobile insurance.
