Saint Elizabeth Home Care in Rhode Island seeks a Skilled Registered Nurse Case Manager to deliver compassionate, one-on-one nursing care in clients' homes. You will perform admissions, revisits, and discharges while coordinating with an interdisciplinary team and using point-of-care devices to support patient needs.

This role emphasizes relationship-building, education, and care planning with strong administrative support.

Qualifications

• Current unencumbered RN license in Rhode Island.
• Minimum of one year nursing experience; home care experience preferred.
• CPR certification.
• Reliable transportation, current clean driver's license and automobile insurance.

Responsibilities

• Provide skilled nursing visits including admissions, revisits, and discharges in patients' homes.
• Assess patients and educate families as part of an interdisciplinary team.

The Skilled Registered Nurse Case Manager - Home Care contributes to our Mission of Caring by providing compassionate direct patient care to our clients in a one-on-one setting in their homes. The Skilled Registered Nurse Case Manager - Home Care conducts skilled nursing visits to include admissions, revisits, and discharges while providing assessments and education to patients. The Skilled Registered Nurse Case Manager - Home Care for Saint Elizabeth Home Care is a part of a collaborative interdisciplinary team with ample administrative support. Our Skilled Registered Nurse Case Manager - Home Care is equipped with point of care devices (iPad), as well as all necessary and requested medical equipment and supplies.
