Qualifications

• Willingness to travel across CT for the majority of the workweek.

Responsibilities

• Liaises with clients and care teams to meet client care needs.
• Develops individualized care plans with clients and families.
• Mentors and trains care staff on policies and standards.
• Ensures accurate documentation of client care and communications.
• Participates in recruitment and staff selection when needed.
